- Chart notes should show the patient cannot transfer safely without assistance and that a lift is required in the home.
- Coverage and eligibility are verified before any equipment ships or bills; a sling (E0621) is ordered separately as needed.
- Coverage varies by payer — DWO and supporting documentation required.
